dc.description.abstractThe x-ray request form is an essential tool for communicating patient data and clinical indication from the referring physicians to radiologic department. This study aimed to evaluate the applied x-ray request form and compare between the applied and standard request form in Khartoum state hospitals at emergency departments in Omdurman, Khartoum and Bahri. This study was evaluated 60 request forms in time from June to July"2014". Patient data and clinical indication on the x-ray request form is vital importance. Requests were evaluate according to standard radiological request form. The main result of the evaluation of the applied x-ray request form filling elements including )pt full name, age, sex, job, pt history, pt condition, clinical indication, LMP, date of exam, previous exam, physician name, and clearness of requested exam, technologist name, size of films ,number of films( were found to be 100%, 33%, 0%, 0%, 18%, 0%, 23%, 0%, 58%, 0%, 95%, 95%, 0%, 0%,0% respectively. The total numbers of filling of the results after collected 20 request forms from emergency departments )Bahri, Khartoum, and Omdurman( are )32%,26,33%,26,33%( respectively. Conclusion of this study that the filling of patient data and clinical indication are vital important in request forms. Regarding to the result it show that the method of filling patient information in x-ray request form by the referring physicians in addition to the form of x-ray request form that applied in Khartoum state emergency departments should be reviewed.en_US
