Abstract: This study was conducted in Sinja hospital in Sinnar state during the period from November 2008 to December 2009, in order to determine the sensitivity and specificity of Histidine Rich Protein-II immunochromatografic test (HRP-II based test ICT) in the diagnosis of P.falciprum malaria. Two hundred malaria suspected individuals, males and females with different age groups were involved in this study after taking their informed consent. Personal data was collected from patients presented with symptoms and sings commonly associated with malaria. Blood samples were collected, for each sample thin and thick blood film were prepared, stained with Geimsa and examined microscopically, part of the sample was used for performance of HRP-II based test (ICT). The results revealed that 109 individuals were positive by blood film. 108 were P.falciprum positive while one sample was found to be positive for P.vivax. HRP-II based test (ICT) detected all P.falciprum smear positive cases with 100% sensitivity. In addition HRP-II based test (ICT) detected one case which was blood film negative but from a treated subject, with specificity of 98.9%. No significant difference in malaria prevalence was detected between both sexes. When fever was related to malaria it was found highly associated with positive cases. Other clinical feature showed no significant relation between headache, vomiting, trembling, abdominal pain, muscle fatigue, loss of appetite, back and joint pain and positive cases except diarrhea which showed variable rate ‫‪of prevalence, with the age group less than 5 year having the highest‬‬ ‫.‪prevalence of diarrhea accompanying malaria‬‬
