Abstract: This study done to know and evaluate the causes and symptoms of the pelvic pain in the first trimester using ultrasound as diagnostic imaging modality. This study conducted at Omdurman maternity & Al Saudi hospitals, in the period from 15 May 2013 to 1 August 2015. In this study Ultrasound machine of Aloka type which has Doppler capability, with a convex probe for abdominal examination and sector one for trans-vaginal and another machine of sonac were used . The frequency of the transducers range between 3.5 -7.5 MHZ. The study was done in 73 women pregnant in the first trimester, their ages range between (15-44years). The results of this study showed that 4 patients (5.5%) their clinical signs are pain only, 5 patients (6.8%)suffering from pain watery discharge,36 patients (49.3%) pain with bleeding and28 patients (38.4%) pain with bleeding and watery discharge. Also the results showed that 13patients (17.8%) with complete abortion, 2 patients (2.7%) ectopic pregnancy, 16 patients (21.9%) incomplete abortion,4 patients (5.5%) normal pregnancy,3 patients (4.1%) inevitable abortion,13 patients (17.8%) missed abortion,8 patients (11.0%)threatened abortion,7 patients(9.6%)blighted ovum,2 patients (2.7%)and 5 patients(6.8%) pregnancy with bleeding.
