Abstract: This analytical hospital-based study was carried out in Omdurman Military hospital and Bashaier teaching hospital in Khartoum, during the period between Septembers to November 2018. the problem of the study is that Vaginal bleeding is an increasing complain among pregnant women in first trimester of pregnancy. The study had done on Patients who had come to ultrasound department for obstetrical investigation. A total of 51 patients were investigated by ultrasound machine in Omdurman military hospital and Bashaier hospital. THE study is aimed to evaluate vaginal bleeding in the first trimester using ultrasound machine, to determine the impact of age, gravidity, medical history, and occupation on the cause of vaginal bleeding. The data had been collected using data collection sheets (questionnaire) and then analyzed using the statistical package for social sciences (SPSS). The result of the study reveals that the most common cause of vaginal bleeding in the first trimester is incomplete abortion (43.1%), followed by threatened abortion (25.5%), complete abortion (19.6%), ectopic pregnancy (3.9%), and lastly the molar pregnancy (2%). The result also show that vaginal bleeding is most common between 20- 30 years of age. The multipara women are the most who have vaginal bleeding in the first trimester, followed by prima gravida and lastly grand-multipara. Housewives suffer from vaginal bleeding in the first trimester more than employers. In conclusion the study found that incomplete abortion has higher incidence among causes of vaginal bleeding in the first trimester (43.1%), while molar pregnancy has the lowest incidence (2%) and of vaginal bleeding in the first trimester.
