Abstract: VIII Abstract The request form is a way to gather information about the patients who undergo CT or any other radiological procedure. The aim of this study was to assess CT Abdomen request form using an ideal request form and compared it to the request forms collected from different hospitals and diagnostic centers in Khartoum state to reduce the complications which can occur due to insufficient information about patient’s condition. A total number of 80 CT Abdomen forms referred from various hospitals and diagnostic centers in Khartoum state were evaluated. As for the information in the forms collected from the hospital under study (80 CT Abdomen request forms), the patient name, gender, age, clinical data, exam date and physician name were included in the request forms. Height, allergy to medications and contrast media, anesthesia, renal function test (GFR) and LMP were not existed. Phone number was found in 95.0 %, furthermore weight was found in 22.5% of request form. Diabetes was found in 11.3% of request form. Creatinine was found in 20.0% and Sedation was found in 7.5%. Recommended CT departments should issue one standard CT Abdominal request form that should be applied by all the hospitals, in order to enhance the CT procedures for better diagnosis and for care of patient. All doctors should follow the standard CT request carefully before referring the patients to the CT department. Finally, I recommend that all departments should not accept patients with incomplete CT Abdominal request form.
